| 概要 |
肺癌は、工業化の進んだ国々において、男女ともにがんによる死亡の主要な原因となっている。 非小細胞肺癌（NSCLC) は、肺癌のおよそ８５％を占めており、主に扁平上皮癌 (SCC), 腺癌 (AC), 大細胞癌 とから構成される。分子メカニズムの変化には、がん遺伝子である K-RAS, EGFR, EML4-ALK の活性化と、がん抑制遺伝子である p53, p16INK4a, RAR-beta, RASSF1などの不活性化が含まれる。K-RASの点突然変異は、GTPase を不活性化させ、p21-RASタンパクは恒常的に増殖シグナルを核に伝える。EGFR の突然変異や過剰発現は細胞の増殖を促進する。EML4 と ALK の融合は ALK を恒常的に活性化させ、これにより細胞の増殖、浸潤、アポトーシスの抑制が引き起こされる。p５３ の不活性化突然変異は細胞の増殖スピードを増大させ、アポトーシスを低下させる。p16INK4a は CDK4 や CDK6 と競合的に結合することにより CDK-cyclin-D 複合体の形成を妨げる。p16INK4a の発現の低下はNSCLC に共通に見られる特徴である。RAR-beta はビタミンA依存に転写活性を増大させる核内受容体である。RASSF1A は、RAS エフェクターである Nore-1とヘテロダイマーを形成することができる。よって、RASSF1A が失われると、RAS の働きは増殖を促進する方に転換されるだろう。
