| 概要 |
乳癌は、女性のがんによる死因の中でもトップを占めるがんである。乳癌の大部分は、乳腺の乳汁を分泌する管を裏打ちする細胞から生じる癌腫である。乳癌の亜型は、ホルモン受容体（エストロゲンとプロゲステロンのサブタイプがある）と human epidermal growth factor receptor-2 (HER2) の存在の有無に基いており、その中にはホルモン受容体陽性 HER2 陰性タイプ (ルミナルA タイプ)、ホルモン受容体陽性 HER2 陽性タイプ (ルミナル B タイプ)、ホルモン受容体陰性 HER2 陽性 (HER2 陽性)、ホルモン受容体陰性 HER2 陰性 (basal-like または トリプルネガティブ乳癌 (TNBCs)) が含まれる。ホルモン受容体陽性乳癌は主にエストロゲン/ER パスウェイにより引き起こされる。HER2 陽性乳癌では、HER2 が PI3K/AKT と RAS/RAF/MAPK パスウェイを活性化し、細胞の増殖、生存、分化を促進する。TNBC の患者では、さまざまなシグナル伝達パスウェイ (Notch や Wnt/beta-catenin) の脱調節や EGFR の発現が確認されている。乳癌全体の中で遺伝性のものは 8% にすぎず、BRCA1 や BRCA2 の遺伝子変異が関係している。全ての乳癌の中で 10% を超える体細胞変異は、TP53、PIK3CA、GATA3 の３種のみである。
