| 概要 |
尿路上皮は、腎盂から尿管、膀胱を経て尿道に到る、尿路のほぼ全体の管腔側を覆っている。尿路上皮癌の大部分は膀胱癌であり、腎盂と尿管に生じる尿路上皮癌は、全体のおよそ７％にすぎない。尿路上皮癌はいくつかの異なる表現型のパスウェイを通じて生じ、発展する。それらの中にはまず、尿路上皮過形成(urothelial hyperplasia)から、低異形度非浸潤性表在乳頭状癌(low-grade non-invasive superficial papillary tumours)へと進展するものがある。より悪性度の高い異形(variant)は、平坦状の高異形度がん(flat, high-grade carcinoma in situ: CIS)から浸潤性のがんへと進展するか、または、最初から浸潤性のがんとして生じるかのどちらかである。低悪性度の乳頭状癌においては、レセプター型チロシンキナーゼーRas パスウェイの恒常的活性化がよく見られるが、これには HRAS と FGFR3 の活性化突然変異が伴う。一方、CIS と浸潤性のがんでは、TP53 と RB の遺伝子やそのパスウェイに高頻度に変化が見られる。浸潤と転移は、腫瘍の微小な環境を変化させる要因によって促進される。これらの中には、E-cadherin, MMP, VEGF などの血管新生に関連する物質などが含まれる。
