T-B- 重症複合免疫不全症

アデノシンデアミナーゼ欠損症 [DS:H02309]

重症複合免疫不全症(SCID)は、ある種の病原体（バクテリア、ウイルス、真菌など）の重篤な感染を早発性に引き起こす、様々な単一遺伝子疾患のグループから成る。SCID の患者においては、T細胞の分化が重篤に損なわれ、B細胞の発生と機能も直接的、間接的に損なわれるのが典型的である。Adenosine deaminase (ADA) deficiency は、常染色体劣性遺伝の SCID のうち、約半数を占めている。これは最も重篤な免疫不全症のひとつであり、B細胞、T細胞、ナチュラルキラー（NK）細胞の欠乏を伴う。 recombinase-activating gene 1 (RAG1)、 RAG2 および Artemis (DCLRE1C)遺伝子の異常により、V(D)J- recombination が正常に行われなくなると、それぞれ pro-B および pre-T 細胞のステージで B細胞とT細胞の成熟が停止することを特徴とする T-B-SCID の表現型が引き起こされる。しかし、この場合ナチュラルキラー (NK)細胞の成熟は影響を受けない。V(D)J recombination はB細胞とT細胞の一次免疫レパートリーの多様性を生じるものである。
