強迫性障害

強迫性障害 (OCD)は、繰り返し不快な考えが頭に浮かんだりや特定の行為を繰り返すことを特徴とする精神疾患である。OCDは複雑な疾患で、その病理は遺伝的、環境的因子両方の影響を受けていると思われる。140以上の候補遺伝子について研究が行われたが、サンプル数が少なく追試がほとんどないため確定的とはいえない。多くの研究で、セロトニン作動性神経伝達の異常がOCDにおける最も一貫した生物学的知見である。また、基底核や中隔側座核におけるドーパミン系の異常もグルタミン酸伝達の異常と同様に報告されている。本疾患の第一選択療法は認知行動療法(暴露・反応妨害法)と選択的セロトニン再取り込み阻害薬 (SSRI) である。最近、OCD患者の治療効果を上げるため開発された新しい有望な治療法の一つにアクセプタンス・コミットメント・セラピー(ACT) がある。最近の研究では、OCDの分類に発症年齢が重要であることが示唆されている。早期発症OCDは遅発性OCDに比べ、症状が重く、チック関連障害の有病率が高く、家族性であることが多く、第一度近親者における精神疾患の有病率が高いと言われている。
