| 概要 |
中咽頭がんは、後部の咽頭側壁、軟口蓋、扁桃部、および舌の後部 1/3 といった解剖学的範囲に生じるがんを含む、総称的な用語である。これらのがんの大部分 (>95%) は、粘膜に由来する扁平上皮がんである。中咽頭扁平上皮がん (OPSCC) は、従来より、頭頸部扁平上皮がん (HNSCC) に分類されている。危険性の高いヒトパピローマウイルス (HR-HPV) が、現在では OPSCC の一部を引き起こす要因として知られている。HPV による OPSCC と、タバコやアルコールによる OPSCC は、生物学的に別個のものである。前者では、p53 と pRb は両方ともウイルスの腫瘍性タンパク質である E6 と E7 によってそれぞれタンパクレベルで不活性化される。一方、HPV によらないがんの方では、TP53 遺伝子の突然変異、epidermal growth factor receptor (EGFR) 遺伝子の増幅、免疫組織化学的に検出される EGFR の過剰発現などが見られる。
